Persons requesting disability services must provide current documentation from a licensed professional who is qualified in the diagnosis of the disability and who is currently or has been recently associated with the individual. The diagnosis must reflect the present level of functioning of the major life activity that is substantially impaired. Documentation must be submitted before obtaining accommodations and services.
If the disability documentation is incomplete or inadequate to determine the present extent of the disability and/or the appropriate accommodations, Disability Services will request supplementary documentation. It is the responsibility of the person requesting accommodations to provide complete and recent documentation.
Upon completion of the verification process, the Learning and Assessment Center Coordinator or a representative of Disibility Services will meet the person with the disability to discuss and accommodation plan.
The Ohio Secretary of State has designated the office that provides disability services at the state-assisted university where you are enrolled to be a "designated agency" pursuant to state and federal laws. This means that the office that provides disability services and the Ironton campus also now offers voter registration services. You may register to vote in Ohio or if you already are registered to vote in Ohio, you may update your registration if you have changed your name or address by obtaining a voter registration form from Disability Services in the Learning and Assessment Center. If you need assistance filling out the voter registration form, someone in the office will be available to assist you.
